Patient Rights & Responsibilities

Patients Have the Right…

  1. To be treated with respect, consideration and dignity.
  2. To be provided with appropriate privacy.
  3. To review their medical records, and to know that patient disclosures and records are treated confidentially and released upon patient authorization, except when required by law.
  4. To know, to the degree known, complete information concerning their diagnosis, evolution, treatment, and prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.
  5. To be given the opportunity to participate in decisions involving their health care, except when such participation is contraindicated for medical reasons.
  6. To obtain information concerning:
    1. Patient Rights.
    2. Patient conduct and responsibilities.
    3. Eligibility for services.
    4. Provisions for after-hours and emergency care.
    5. Policies for fee-for-service.
    6. Payment policies.
    7. Patient’s right to refuse to participate in experimental research.
    8. Advanced directives, as required by state or federal law and regulations.
    9. Provider Credentials.
  7. To change provider if other qualified providers are available or to request the same provider.
  8. To know that marketing or advertising regarding the competence and capabilities of the organization accurately reflects services provided.
  9. To know that the provider staff is insured under the State of Florida Self-Insured Program.
  10. To express grievances and suggestions to University Health Services through the use of an evaluation form or by meeting with the Patient Advocate at the University Health Services.

Patients Have the Responsibility…

  1. To provide complete and accurate information to the best of their ability to the provider about present complaints, past illnesses, hospitalizations, medications including over-the-counter products and dietary supplements, allergies and sensitivities and other matters that are related to their health.
  2. To follow the plan of treatment as discussed and recommended by the health care provider.
  3. To obtain transportation by a responsible adult who will provide 24 hour care if required.
  4. To provide information about a living will, medical power of attorney, or any other directive that could affect medical care.
  5. To accept responsibility for any charges related to their care.
  6. To treat the staff with respect, consideration and dignity.
  7. To request care and treatment from a health care provider who is not responsible for grading their academic performance.
