Immunization FAQs


Why do I have an immunization hold on my.fiu.edu?
If a student has an immunization hold, he or she may not have provided proof or submitted insufficient proof of immunity to MMR, Hepatitis B and Meningitis. In order to remove a hold, the student would need to provide the Health Compliance unit with the respective documents. All documentation will be processed within two business days (48 hours) of acceptance. To check the status of your current Immunization compliance, please visit the holds section on https://my.fiu.edu/

How do I electronically accept immunization waivers?
Students declining to receive vaccination for Meningitis and/or Hepatitis B must accept awaiver of liability acknowledging that they have received and read information pertaining to the disease and despite knowledge of the risks have decided to waive receiving the vaccine.

Immunization waivers can be accepted and viewed on my.fiu.edu under the “Student Center” and “Student Health Portal” tabs. Select the “Immunization Waivers” link to either “accept” or “cancel” (not accept) the waiver.

If a student is under the age of 18, the waiver of liability must be signed by a parent or legal guardian. Waivers can be located in the “Forms” section of this website.


What are the acceptable forms of documentation?
The following documents are acceptable proofs of immunity for all vaccines, provided that the dates are suitable and the documents are signed and stamped by the health care provider:
  • FIU Immunization Documentation Form
  • Health Department records
  • Childhood immunization records
  • School immunization records
  • Military service records
  • Documentation of having the disease during his or her lifetime: Applies to Measles (Rubeola) and Mumps only
  • Laboratory blood test (serology) results demonstrating immunity:
    • Measles (Rubeola): positive IgG titer required
    • Rubella: positive IgG titer required
    • Hepatitis B: positive AB antibody surface titer required
    • Meningitis: a lab test is not valid and will not be accepted

I cannot find my records, what should I do?
If a student cannot locate his or her immunization records and are confident they have taken the vaccines, a positive lab test against both the Measles (Rubeola) and Rubella and/or Hepatitis B is sufficient. Laboratory blood tests with equivocal or negative results will not be accepted. Additionally, students can always choose to be revaccinated in cases of uncertainty.

When do I need to submit immunization records?
Students should submit immunization records preferably at least four (4) weeks before you register. We recommend fulfilling this requirement as soon as possible to minimize delays when registering. The month prior to registration before each semester are very busy times for the Health Compliance Unit so early submissions are greatly appreciated.

How and where do I submit immunization records?
Copies of your immunization records can be submitted to FIU Student Health Services via mail, fax or email. Please include your Panther ID number on all documents and allow 24-48 hours for processing of your immunization documents. After a document hasbeen submitted, you may check the status of your Immunization compliance at https://my.fiu.edu/.

Can I take vaccines on campus?
Yes! Students can receive the vaccines on a walk-in basis (no appointment required) at one of FIU's Student Health Services MMC or BBC during our regular hours of operation.

I am currently pregnant, should I be vaccinated?
If a student is pregnant, breast feeding, trying to get pregnant or undergoing fertility treatments, vaccination is not recommended. She can temporarily waive the immunization requirements until birth or breast feeding has ceased. Proper
documentation consists of a Personal Physican Vaccine Waiver Form or a letter from a doctor’s office with the following information:
  • The date the letter was provided
  • Brief explanation for waiving the vaccine
  • The term for which a student will need to be exempt from taking the vaccines
  • The students panther ID number
  • An M.D. stamp and original signature

This letter will only be valid for one semester unless the letter specifically states an expiration date. Once the expiration date has passed, the Immunization holds will be automatically placed on the students account.


I am currently ill, should I be vaccinated?
If a student is temporarily ill or involved in any medical circumstance, it is not recommended for a student to take any vaccines. Proper documentation consists of the Personal Physician Vaccine Waiver form or a letter from a doctor’s office with the following information:
  • The date the letter was provided
  • Brief explanation for waiving the vaccine
  • Date in which a student is able to take a vaccine
  • The students panther ID number
  • An M.D. stamp and original signature

This letter will only be valid for one semester unless the letter specifically states an expiration date. Once the expiration date has passed, the Immunization holds will be automatically placed on the students account.


I’m a transient student; do I need to submit records?
Transient students from a public university in Florida have the option to submit Immunization documents or provide the Health Compliance Unit with a copy of his or her transient form verifying that the student’s home school has the Measles and Rubella vaccines on file. This option will lift the student’s MMR (Immunization) hold temporarily for one semester. Transient students need to provide documentation regarding Hepatitis B and Meningitis via proof of vaccination or waivers. If a student continues taking classes as a transient student, he or she will need to submit proof each semester. A student’s home school can also send the Health Compliance unit a copy of the original Immunization record.

I’m taking both online and campus classes; do I need to submit records?
Yes. A hybrid student is a student who is enrolled in a program that combines both oncampus and off-campus learning via online classes. Any student that will be physically visiting any FIU campus is required to submit Immunization documents to the Student Health Services.

I’m taking classes at the Broward Pines Center; do I need to submit records?
Yes, students attending classes at the Broward Pines Center must submit immunization records. Below is the contact information for additional assistance:

My program is fully online. Do I need to submit records?
If a student is not physically coming to one of FIU’s campuses, he or she is still required to show proper documentation to the Health Compliance Unit. They can choose to submit Immunization documents for MMR, Hepatitis B and Meningitis or provide a letter from their academic department. The letter must have the following:
  • Must be written on department letterhead paper
  • Must include the students panther ID number
  • Must state the specific academic program
  • Must verify that the student is enrolled in a fully online program
  • Must state the length of the program
  • Must be signed by the FIU employee representing the academic program

This letter will only be valid for one semester unless the letter specifically states an expiration date. Once the expiration date has passed, the Immunization holds will be automatically placed on the students account.


My program is a Distant Learning Program. Do I need to submit records?
If students are part of a program that takes place outside of FIU’s campuses, students have the option of waiving the immunization requirements by providing the Health Compliance Unit with a letter from the respective program. This letter must have the following:
  • Must be written on department letterhead paper
  • Must include the students panther ID number
  • Must state the specific academic program
  • Must verify that the student is enrolled program that takes place off campus
  • Must state the length of the program
  • Must be signed by the FIU employee representing the academic program

This letter will only be valid for one semester unless the letter specifically states an expiration date. Once the expiration date has passed, the Immunization holds will be automatically placed on the students account.


How do I request a permanent medical exemption?
Permanent exemptions are given to students who have a permanent medical conditionin which vaccinations are not recommended. Proper documentation consists of the Personal Physician Vaccine Waiver Form or a letter from a doctor’s office with the following information:
  • The date the letter was provided by M.D.
  • Brief explanation for waiving the vaccine permanently
  • The students panther ID number
  • An M.D. stamp and signature

How do I request a permanent religious exemption?
Permanent exemptions are given to students who provide documentation regarding religious beliefs and/or practices against vaccinations. A religious exemption applies to MMR only. Proper documentation consists of one of the following:
  • Completion and submission of the “Vaccine Waiver for Religious Exemption” form and a letter from the student’s religious institution. The letter must report that the student is an active member of the religious institution and in good standing. It must also report that it is the tenet or practice of that religious institute that its members not receive medical vaccinations. No details of the religious tenet or practice are required. The letter must be written on the religious institution’s letterhead and signed by the officiating clergy, and it must include his/her address and telephone number.
  • If a student is not a member of an organized church or religious sect, or if thestudents personal religious beliefs are not shared by the religious institution, a student can complete the “Vaccine Waiver for Religious Exemption” form and provide a letter stating that his or her personal religious beliefs and practices do not permit him or her to receive medical vaccinations.
