International Student and Graduate Student Insurance Information

Medical Insurance Policy For International Students

By Board of Trustees and FIU rule, all international students with F-1 and J-1 status are required to have medical insurance prior to registration. Proof must be submitted to the Health Compliance Office at University Health Services. Students will NOT be allowed to register for classes without complying with the medical insurance requirement. They will receive an "IMI" service indicator (hold) on their student record, as it appears in Panthersoft.

The medical insurance requirement can be met in one of two ways:

All insurance related forms can be found on this page, as well as in the Forms Library.

Alternative Insurance Policy

If you have your own insurance policy (or wish to purchase a policy other than the University-approved policy), your insurance company must complete and fax the International Student Alternative Health Insurance Compliance Form to UHS. NOTE: The UHS will not be able to review any alternative health insurance plan without the Compliance Form. The guidelines for insurance have been established by the Florida Board of Governors and must be met. Please review the Florida Board of Governors state regulation prior to purchasing any alternative plan.

The completed compliance form must verify the following:

  • the coverage meets or exceeds the minimum FIU requirements
  • the name, address, and telephone number of a claims agent in the United States
  • you are covered under the policy
  • your insurance has been paid through current year

The Health Compliance Unit must receive the International Student Alternative Health Insurance Compliance Form directly from your insurance company. Your policy will be reviewed, and if it complies with the guidelines, you will be cleared to register. If your policy does not comply with the guidelines, you will not be cleared until you document insurance coverage which meets FIU requirements or purchase the University-approved insurance policy.

Medical Evacuation & Repatriation

If your alternate policy is approved subject to Medical Evacuation & Repatriation, the service indicator will not be released until this requirement is met. You may purchase coverage for Medical Evacuation & Repatriation from Collegiate Risk Management at UHS with a money order or cashier's check in the amount of $40.00 payable to Collegiate Risk Management.

Note: You should NOT purchase Medical Evacuation and Repatriation UNTIL you have proof that your alternate insurance policy was approved subject to it.

Online-Payments

Credit card payments are accepted through Visa and MasterCard only via the Collegiate Risk Management Website. Scroll down to the section that reads “International Students, Visiting Scholars, Exchange Students and English Language Institute Students: International (Regular) Students” and select Enroll Online. Once you have submitted the application and your payment clears, you will receive a confirmation indicating that the credit card transaction was successful. Print this page and keep it for your records.

A copy of this confirmation can be submitted to our office to remove your hold immediately. Please include your Panther ID if submitting by fax.
